Vitamin D deficiency is one of the most common nutritional problems in India, affecting anywhere from 60% to 90% of the population according to various studies — and that figure holds true even in sunny regions like Panchkula and Chandigarh. It is often called a silent deficiency because the symptoms develop gradually and are easy to dismiss as stress, ageing or simply feeling run down. A simple blood test can confirm it, and in most cases treatment is straightforward and effective.

Why Is Vitamin D Deficiency So Common in India?

The irony is hard to miss: India has sunshine for most of the year, yet vitamin D deficiency is widespread. Several factors explain this. Vitamin D is made in the skin when it is exposed to UVB rays from the sun, but many people spend most of the day indoors — in offices, classrooms or at home — and when they do go out they often cover up or use sunscreen, both of which block UVB. Darker skin requires more sun exposure to produce the same amount of vitamin D as lighter skin. Air pollution, especially during the post-monsoon and winter months in North India, can also reduce the UVB radiation that reaches the ground. Finally, the Indian diet — particularly vegetarian diets — contains very little vitamin D naturally, and fortified foods are not yet widely available.

Symptoms to Watch For

Most people with low vitamin D have no obvious symptoms at first, or the symptoms are vague enough to be blamed on other causes. Common signs include persistent fatigue that does not improve with rest, generalised body ache or dull bone pain (especially in the legs, hips or lower back), muscle weakness or cramps, low mood or irritability, and getting colds or respiratory infections more often than usual. Hair thinning and slow wound healing are also reported. In children, severe deficiency can lead to rickets, a condition that affects bone development. In adults, long-term deficiency increases the risk of osteomalacia (soft bones) and osteoporosis. If several of these symptoms sound familiar, it is worth asking your doctor for a blood test.

How the Test Works

The blood test measures a form of vitamin D called 25-hydroxyvitamin D, or 25(OH)D. A level below 20 nanograms per millilitre is considered deficient, 20 to 30 ng/mL is insufficient, and above 30 ng/mL is the target range for most adults. Severe deficiency is defined as a level below 10 ng/mL and requires more urgent treatment. The test is widely available at diagnostic centres across Panchkula and the Tricity area, and results are usually ready within a day or two.

Treatment: How Cholecalciferol Supplements Work

Treatment for vitamin D deficiency typically involves cholecalciferol, which is vitamin D3. It is available as tablets, capsules and sachets. The standard treatment protocol in India for confirmed deficiency is a higher dose for several weeks to rebuild stores, followed by a lower maintenance dose to keep levels stable. A commonly prescribed regimen is 60,000 IU once a week for eight weeks, followed by a monthly or fortnightly sachet, or a daily dose of 1,000 to 2,000 IU depending on the person's weight, baseline level and other factors. The loading phase usually takes eight to twelve weeks, and a retest is done at the end to confirm that the level has improved.

It is important not to take high doses on your own without a blood test and a doctor's advice. More is not always better, and taking very high doses for a long time without monitoring can raise calcium levels in the blood to unsafe levels, though this is rare. Cholecalciferol works best when taken with a meal that contains some fat, as vitamin D is fat-soluble. If your diet is low in calcium — which is common in India — your doctor may also recommend a calcium supplement, because vitamin D helps the body absorb calcium but cannot replace it.

Sunlight and Diet

While supplements are the most reliable way to correct a deficiency, getting some safe sun exposure can help maintain levels once they are back to normal. About 15 to 20 minutes of sunlight on the arms and legs, two to three times a week, in the late morning or early afternoon (when UVB is present but the heat is not too harsh) is a reasonable target, though this varies by skin tone, season and pollution levels. Sitting by a window does not count, as glass blocks UVB rays.

Very few foods naturally contain vitamin D. The best sources are fatty fish like salmon and mackerel, egg yolks, and fortified milk or cereals. For most people in India, diet alone is not enough to meet the body's needs, which is why supplementation is often necessary.
